We are a referral-based periodontic clinic.

Dental professionals, please complete our intake form below and submit it by email, fax or mail.

Patients, when your dentist determines that specialized treatment is required related to your gum health and the supporting structures of your teeth, they will provide a referral to our clinic. If you feel our services are right for you, ask your dentist to refer you to our clinic. After receiving your referral, our team will contact you to schedule your intake appointment.
